Sit beside them, not standing. Soft smile. If they reach for your hand, gently hold with both of yours. Don't dwell long—they need time to prepare for departure too. Leave with light wave and bow at the door.
Last-day farewell from caregiver to long-term resident. 「少し寂しくなります」 (it'll be a bit lonely) is appropriate genuine emotion after building relationship—don't suppress it, but don't burden them either. 「本当にありがとうございました」 thanks for their patience/trust. Hand a small handwritten card if appropriate.
