Sit at eye level, notebook ready. Listen attentively, take notes of their concerns. Repeat back to confirm understanding ('夜眠れないこと、お伝えしますね'). Schedule the nurse visit at a time they're alert.
Coordinating with visiting nurses (訪問看護師) is essential for home-bound seniors. Always ask the senior for their concerns BEFORE the nurse arrives — gives them a voice in their own care. Pass concerns clearly to the nurse with timing/severity. Take notes during the nurse's visit and review with the senior afterward.
