Stand when delivering this in person. Bow at 45 degrees. Keep the explanation brief — one sentence on the error, one on the remedy. Do not over-explain or list mitigating factors until the superior asks.

In Japanese workplace culture, reporting your own mistake immediately (before it is discovered) is considered far more honourable than having it found out. Pair the apology with a concrete action statement ('I will address it immediately') to show accountability, not just regret.
