Choose a private moment away from the patient for this conversation. Maintain a calm, measured tone — not alarmed, not dismissive. Have your observation notes available so you can share specifics if the family asks for more detail.
Framing an observation as a question 「お気づきでしたか?」brings the family into the conversation as partners rather than passive recipients of information. 「〜ているようで」(it seems that...) softens the observation without minimizing it. Always follow up by asking 「何か思い当たることはありますか?」(Is there anything that comes to mind?).
