When reporting a mistake to a superior, the structure 'apologize → state the issue → state immediate corrective action' is essential. Do not over-explain or offer excuses — that compounds the offense. 'Moushiwake gozaimasen' is more weighty than 'sumimasen' for actual errors. 'Tadachi ni' (immediately) shows urgency.
