Place the medication and water cup within easy reach, both at once. Stay nearby until the medication is taken rather than walking away immediately. If the person has difficulty swallowing tablets, ask: 'Nomi komi nikui desu ka? Suibu wo ooku shimasu ka?' (Is it hard to swallow? Shall I give you more water?). Record the administration time in the care record.
Medication reminders should be delivered as a matter-of-fact routine, not with the emphasis or urgency of an alarm. Presenting the water alongside the medication as part of the offer ('kochira no omizu to issho ni douzo') makes it a single smooth action rather than two sequential steps.
