Always verbalize each step before doing it. Wheelchair brakes locked—check twice. Face them directly, feet shoulder-width apart for stability. After successful transfer, slip a small cushion behind their back for comfort. Adjust footrests after they're seated.
Transfer to wheelchair—high fall risk maneuver. Count synchronizes effort. Lock wheelchair brakes first. Position chair at 30-45 degree angle to bed. Have them push from armrests, you support under armpits, knee blocks their knee (prevents collapse). Pivot in one smooth motion. Praise: 「上手にできましたね」.
