Maintain steady, calm eye contact as you speak; ensure they are in a stable comfortable position; ask 'お手に触れてもいいですか?' (May I touch your hand?) before any physical contact; narrate each step of the procedure to keep them informed.
Preparing seniors for discomfort before it happens dramatically reduces anxiety and improves tolerance. Use 'かもしれません' (might) rather than 'あります' (will be) to soften the warning. Always follow with: 'もし辛くなったらすぐ教えてください' (Please tell me right away if it becomes too much) to give them a sense of control.
