Calm, professional tone. If they struggle with numbers, demonstrate gently: '0 ga itami nashi, 10 ga sugoku itai desu' (0 is no pain, 10 is extreme). Hold up fingers to visualize. Document promptly. If the score is concerning, escalate to medical staff without alarming the senior.
Pain scales (NRS — Numerical Rating Scale) help quantify subjective sensation. Many seniors find numerical answers difficult initially; offer alternatives ('hodohodo desu ka? hidoi desu ka?' = is it moderate or severe?). Document the score and re-assess at intervals. Never tell them what score they should give based on visible behavior — let their report stand.
