Calm conversational tone, not investigative. Sit at eye level, give them time to think before answering. Have a small notebook ready to document their words verbatim. Smile reassuringly while listening — they may worry that any change means bad news.
After medication changes, proactive symptom monitoring catches side effects early. Use open language ('nanika henka' = any changes?) rather than leading questions ('zutsuu wa arimasen ka?' = no headache?) which can plant suggestions. Document responses literally, even if vague. Pass concerning observations to medical staff promptly. Never reassure with 'sore wa kusuri to kankei nai desu' (that's unrelated to the medicine) — let professionals interpret.
