Gesture & etiquette
Hand medication cup with both hands, water cup separately. Watch them place pills in mouth, drink water, and swallow (visible throat motion). Check inside mouth gently if uncertain ('お口の中、見せていただけますか'). Document.
Medication reminder. 「お水と一緒に」 specifies water (not tea, juice, or grapefruit which can interact with drugs). Watch them actually swallow—do not leave the room until you've confirmed. Note in chart any refusal or difficulty. Some meds have specific timing (before/after meals)—check the schedule.
