Point to the menu item, then make a 'removing' motion with hands if necessary, or just state clearly.
Be specific about the ingredient. If it's a severe allergy, it's better to state it clearly (アレルギーです: arerugī desu). Understand that not all requests can be accommodated, especially in small kitchens or for pre-made items.
